Forzani Foundation
The Forzani Foundation is a non-profit corporation incorporated under the Canada Corporations Act. It provides financial support to other registered charities, Canadian amateur athletic associations and other qualified donees whose activities involve the promotion of physical fitness; health and wellness; the prevention and relief of sickness and disability; or the participation and education in sports across Canada.

Funding for the Foundation is provided through annual events such as the Mother’s Day Run & Walk in Edmonton & Calgary as well as a number of other fundraising initiatives and general donations throughout the year.

This year, the Mother’s Day Run & Walk will be hoping to raise over $220,000
at an estimated cost of $70,000.

Stollery Children's Hospital Foundation
The Stollery Children’s Hospital Foundation is dedicated to raising funds for specialized equipment, sub-specialty medical education to train the best of the best, research to pave the way to the discovery of new treatments or cures for child health issues, and specialized programs that improve patient and family outcomes at the Stollery Children’s Hospital. This year, money raised at the Sport Chek Mother’s Day Run & Walk will help purchase two critical pieces of equipment together worth $102,000.

The Optical Coherence Tomography (OCT), is a non-contact medical imaging technology similar to ultrasound and MRI. With OCT, reflected light is used to produce detailed cross-sectional and 3-D images of the eye at 40,000 scans per second which results into higher resolution and a better result in observing disease at the back of the eye; and will allow early detection and treatment before vision is lost.

The Non-Mydriatic Fundus Camera is a table-based digital camera unit for photographing features of the back of the eye. This device is a useful tool to help doctors to decide on medical or surgical interventions in not only ophthalmology, but also neurology and neurosurgery. Acquiring a suitable Non-Mydriatic Camera for children will increase the scope and quality of the care provided.

Raise funds and you will receive one of the following items based on the amount of pledge money you turn in. Prizes are not cumulative. Product may not be the same as shown and may be replaced with product of equal or greater value.

Fundraising Instructions

Please include the full name, address, postal code, phone number and pledge amount for each donor; otherwise a tax receipt cannot be issued. Tax receipts will only be issued for amounts of $20 or more and will be mailed prior to December 31, 2013.
All cheques payable to: Forzani Foundation
Completed pledge forms and collected monies must be turned in at the (Individual & Team Package Pick Up) on Friday, May 10 @ 10:00am - 8:00pm or Saturday, May 11 @ 10:00am - 2:00pm at Sport Chek - South Park Centre (Unit 130, 3803 Calgary Trail S.)
Pledge Prize item will be based on the total amount of Pledges turned in at the Package Pick Up (No Exceptions!)
Pledge Prizes will be available until June 14, 2013. Pledges handed in after this date will no longer be eligible to receive pledge prizes.
