Forzani Foundation

The Forzani Foundation is a non-profit corporation incorporated under the Canada Corporations Act. It provides financial support to other registered charities, Canadian amateur athletic associations and other qualified donees whose activities involve the promotion of physical fitness; health and wellness; the prevention and relief of sickness and disability; or the participation and education in sports across Canada.

Funding for the Foundation is provided through annual events such as the Mother’s Day Run & Walk in Calgary & Edmonton as well as a number of other fundraising initiatives and general donations throughout the year.

This year, the Mother’s Day Run & Walk will be hoping to raise over $720,000
at an estimated cost of $245,000.

Calgary Health Trust

Every infant deserves a fighting chance to live the best life possible. Your support of the Sport Chek Mother's Day Run & Walk helps to provide the resources to give them just that.

With access to state-of-the-art neonatal equipment, clinicians are able to provide specialized care to these infants - and in many cases, prevent longer term developmental issues. As a major supporter of advancing health care in Calgary, the Forzani Foundation is again directing a portion of the proceeds raised to Calgary Health Trust in support of the neonatal units throughout our city.

Calgary has one of the highest rates for premature and multiple births in the country. One in ten babies born in Alberta this year will be born before 37 weeks. Through your support, we are able to keep learning about better, more effective ways for care and maximizing the resources for these tiny patients.

This year, the funds from the Sport Chek Mother's Day Run & Walk will purchase equipment that is an essential part of the evaluation and delivery of care; from EEG, ECG, ultrasound machines, which help in the monitoring of key vital signs, to customized neonatal beds and lights to help in development and overall health of the smallest patients.

Raise funds and you will receive one of the following items based on the amount of pledge money you turn in. Prizes are not cumulative. Product may not be the same as shown and may be replaced with product of equal or greater value.

Fundraising Instructions

• Please include the full name, address, postal code, phone number and pledge amount for each donor; otherwise a tax receipt cannot be issued. Tax receipts will only be issued for amounts of $20 or more and will be mailed prior to December 31, 2013.

• All cheques payable to: Forzani Foundation

• Completed pledge forms and collected monies must be turned in at the Team Package Pick Up on Thursday, May 9 @ 12:00pm – 8:00pm or the Fitness Fair (Individual Package Pick Up) on Friday, May 10 @ 12:00pm – 8:00pm or Saturday, May 11 @ 9:00am – 2:00pm at Max Bell Centre (1001 Barlow Trail SE).

• Pledge Prize item will be based on the total amount of Pledges turned in at the Team Package Pick Up or Fitness Fair (No Exceptions!)

• Pledge Prizes will be available until June 14, 2013. Pledges handed in after this date will no longer be eligible to receive pledge prizes.
